If Adam is the first mutant, why is he so much younger than everybody in the 'previous' generation, and why weren't there any other mutants during the 400 years between his appearance and the current mutants?

Well, he stopped ageing after his regen kicked in. The one way he could be the first mutant is if Hiro's time traveling somehow caused Adam's mutation, but that's a bit hard to swallow.
